New Delhi:

The Directorate General of Civil Aviation (DGCA) has issued show cause notices to four top IndiGo officials after a special audit team of the civil aviation watchdog found multiple safety lapses, according to a report by PTI news agency, citing sources.

"Captain Sanjiv Bhalla, Head of Training; Captain Hemant Kumar, Chief of Flight Safety; Captain Ashim Mitra, Senior Vice President - Operations; Captain Rakesh Srivastava, QA (Quality Assurance) and Ops Safety - these four executives of IndiGo have been issued show cause notices today," the agency quoted a source as saying.

The Directorate General of Civil Aviation had carried out a special audit at the IndiGo office in Gurugram (Gurgaon) on July 8 and 9. During the audit, the team of DGCA found several safety and security lapses in the airlines.

Following multiple landing incidents across the country, the DGCA constituted a special team to audit all airlines and airports which are in monsoon-affected areas.

Last month, A Guwahati-bound IndiGo Airlines flight from Hyderabad made an emergency landing at the Biju Patnaik International Airport in Bhubaneswar. On July 2, a SpiceJet aircraft from Pune veered off towards right from the runway centre line at the Kolkata Airport, owing to wet runway and a heavy rainfall during landing.
